Fill in Parts 1, 2 and 4 if you are an officer or authorised employee of a union that has lodged a Form F23G and you need to lodge a declaration in support of that application.

Fill in Parts 1, 3 and 4 if an employer has lodged a Form F23F and you need to lodge a declaration about that application.

Before you start

Have this information ready before you begin. You will need to tell us:

the number of employees employed by the employer
the industry of the employer

You must send (serve) a copy of your declaration to each employer and employee organisation that is covered by the agreement.
