Who can use this form
Use this form is if you are:
- a registered organisation (like a union), or
- a regulated business
that gave the consultation notice to the receiving entity for the proposed collective agreement.
Before you start
Have this information ready before you start. You will need to give us:
- contact details for the negotiating entities
- a copy of the consultation notice.

Lodge this form, with a copy of your consultation notice, on the same day that you provide the notice to the receiving entity.
