If you are an existing organisation, you may use Form F66 where you have not chosen to use a particular ballot paper for a proposed amalgamation and there is a proposal for an alternative amalgamation.

Before you start
The organisations involved in the amalgamation (and any associations proposed to be registered as organisations under the amalgamation) must jointly lodge with the Commission an application for approval for the submission of the amalgamation to ballot.
If there is a provision for an alternative amalgamation, the Commission must be notified of the particulars of:
- the differences between the proposed amalgamation and any proposed alternative amalgamations
- the differences between the rules of any association proposed to be registered as an organisation, and any proposed alteration of the rules of any existing organisations, under both the proposed amalgamation and any alternative amalgamation.
If the proposed amalgamation is not approved by each of the members of each of the organisations, the members of the organisations subject to the alternative amalgamations can vote on the alternative amalgamations.
